Post-acute COVID-19 conditions, commonly referred to as Long COVID, have emerged as a persistent and heterogeneous public health challenge with significant geographic variability across populations. Despite increasing recognition of its clinical and socioeconomic burden, limited analytical frameworks exist for systematically evaluating spatial disparities in symptom prevalence and severity across U.S. states and metropolitan regions. This study develops a multilevel analytical framework integrating epidemiological surveillance, socioeconomic stratification, and health system capacity indicators to assess geographic inequalities in Long COVID outcomes. Drawing on national surveillance datasets and prior empirical evidence, the framework conceptualizes Long COVID as a layered outcome influenced by individual-level infection severity, community-level vulnerability, and structural determinants of health.

The findings synthesized from existing literature highlight substantial disparities in Long COVID prevalence, with higher burdens observed in socioeconomically disadvantaged and geographically diverse regions. Evidence suggests that post-acute sequelae are strongly associated with pre-existing health inequities, occupational exposure risks, and uneven access to healthcare resources. The study further emphasizes the need for standardized surveillance mechanisms and integrated data systems to capture long-term outcomes. As highlighted in national research planning discussions, the current response to Long COVID remains insufficiently coordinated and requires stronger analytical infrastructure to guide policy decisions (Abbasi, 2022).

The proposed framework provides a scalable model for understanding spatial heterogeneity in post-COVID conditions and supports targeted intervention strategies. Ultimately, this work contributes to advancing population health analytics by integrating multilevel determinants into a unified geographic disparity assessment model.

Background: The pancreas is a morphologically complex parenchymal organ composed of closely integrated exocrine and endocrine components responsible for digestive enzyme secretion and hormonal regulation of metabolism. Nicotine is a well-known toxic agent with cytotoxic, pro-oxidant, and vasoactive properties that can disrupt cellular homeostasis and induce structural damage in various organs, including the pancreas. Oxidative stress, apoptosis, and stromal remodeling are considered key mechanisms underlying nicotine-induced tissue injury. However, age-related morphofunctional changes in the pancreas under nicotine exposure remain insufficiently studied. Objective: To compare morphological and morphometric changes in the pancreas of 15-month-old rats under modeled nicotine intoxication with those of the control group. Materials and methods: The experimental study was conducted on 30 outbred white rats divided into two groups: control (n=15) and nicotine-exposed (n=15). Nicotine intoxication was modeled using passive tobacco smoke exposure in a 0.3 m³ glass chamber based on A.S. Solomin (2011) for 30 days, twice daily for 30 minutes. Histological examination of pancreatic tissue was performed using Hematoxylin-Eosin and Van Gieson staining. Morphometric analysis included assessment of exocrine and endocrine parenchymal areas, acinar dimensions, duct diameter, islet size, and cellular characteristics. Results: Nicotine exposure caused significant structural alterations in both exocrine and endocrine components of the pancreas. The relative area of exocrine parenchyma decreased, along with reductions in acinar diameter, epithelial height, and exocrinocyte size and number. The nucleus-to-cytoplasm ratio increased, indicating cellular stress and functional impairment. In the endocrine portion, reductions in islet number, size, endocrinocyte density, and cellular area were observed, reflecting decreased hormonal activity. Histological analysis revealed epithelial desquamation, nuclear swelling, chromatin dispersion, cytoplasmic shrinkage, connective tissue hypertrophy, and interstitial space formation. Van Gieson staining confirmed stromal fibrosis and tissue degeneration. Conclusion: One-month nicotine intoxication in 15-month-old rats induces pronounced morphological and morphometric alterations in the pancreas. These changes include suppression of exocrine and endocrine activity, structural disorganization, connective tissue remodeling, and cellular degeneration. The findings indicate that nicotine significantly impairs pancreatic structure and function through mechanisms associated with oxidative stress, apoptosis, and stromal fibrosis.

Uterine bleeding, particularly postpartum hemorrhage, remains one of the leading causes of maternal morbidity and mortality worldwide. Rapid recognition and effective management of obstetric bleeding are essential competencies for future healthcare professionals. However, traditional medical education often provides limited opportunities for students to practice emergency obstetric interventions due to concerns about patient safety. Simulation-based education has emerged as an effective approach for teaching complex clinical skills in a safe and controlled environment. The aim of this study was to evaluate the effectiveness of simulation-based training in improving medical students' competence in the management of uterine bleeding. A prospective educational study was conducted at the simulation training center of Tashkent State Medical University during the 2025–2026 academic year. A total of 100 senior medical students participated in the study and were divided into two groups: a simulation training group (n=50) and a control group (n=50) receiving traditional clinical instruction. The simulation program included structured obstetric emergency scenarios focusing on uterine bleeding management, postpartum hemorrhage control, patient assessment, and teamwork in emergency care. Students' competencies were assessed using Objective Structured Clinical Examination, instructor evaluation checklists, and pre- and post-training knowledge tests. Statistical analysis was performed using descriptive statistics and Student's t-test, with statistical significance set at p < 0.05. Students who participated in simulation-based training demonstrated significantly higher clinical competence scores compared with the control group. The mean Objective Structured Clinical Examination score increased from 56.8 ± 7.4 before training to 86.2 ± 6.1 after simulation training (p < 0.001), while the control group showed only moderate improvement (57.1 ± 7.2 to 69.4 ± 6.9). Simulation-trained students also demonstrated better performance in recognizing uterine bleeding, initiating emergency management, and coordinating team-based care. Simulation-based training significantly improves medical students' competence in managing obstetric emergencies such as uterine bleeding. Integration of structured simulation programs into medical curricula may enhance clinical preparedness and contribute to improving maternal healthcare outcomes.
