The rapid proliferation of cloud computing in enterprise domains such as retail, finance, healthcare, and critical national infrastructure has elevated concerns regarding security assurance, regulatory compliance, and systems resilience. Responding to this digital transformation, the discipline of DevSecOps has emerged as an integrative paradigm that embeds security principles throughout the software development lifecycle (SDLC), from early design to continuous operations. This research critically examines the theoretical foundations, practical implementations, and empirical outcomes associated with DevSecOps adoption, with particular emphasis on cloud‑native environments and compliance‑driven contexts. Our analysis synthesizes diverse perspectives including comparative evaluations of DevOps versus DevSecOps (Chawla & Malhotra, 2019), enterprise‑scale implementation hurdles (Carlson & Patel, 2020), and principles for secure software engineering (Singh & Roy, 2018). By integrating critical theoretical debate with practical insights, we identify the conditions under which DevSecOps yields demonstrable improvements in cloud security posture, compliance readiness, and operational resilience. Implications for future research and practice are discussed, particularly regarding emerging cloud service models, regulatory dynamics, and the evolving role of artificial intelligence in security automation.

Cervical cancer remains a leading cause of cancer morbidity and mortality among women in Nigeria, despite the availability of effective preventive interventions such as Pap smear screening and human papillomavirus vaccination. Empirical evidence increasingly suggests that disparities in prevention uptake are shaped less by individual attitudes alone and more by structural factors that influence access to information, services, and decision-making capacity. Drawing on data from a cross-sectional study of 402 women attending primary health centres in Nnewi-North Local Government Area, Anambra State, this paper examines educational attainment as a key determinant of knowledge, attitudes, and preventive behaviour related to cervical cancer. Education emerged as the most consistent and statistically significant predictor of awareness of cervical cancer, understanding of Pap smear screening, knowledge of HPV vaccination, and willingness to recommend vaccination. In contrast, age, parity, sexual activity, and marital status showed limited or no association with preventive behaviours. The findings highlight education not merely as an individual attribute, but as a structural enabler of health literacy, agency, and engagement with preventive services. The paper argues that cervical cancer prevention strategies in Nigeria must move beyond awareness campaigns alone and adopt education-sensitive approaches that address entrenched inequalities in access to information and care. Integrating cervical cancer prevention into routine primary healthcare, alongside targeted community education for women with lower educational attainment, is essential for reducing persistent gaps between awareness and practice.

Ultimately, this research contributes to a comprehensive understanding of the automation-driven QA paradigm shift, offering strategic guidance for organizations seeking to harmonize legacy systems with cutting-edge AI capabilities while maintaining rigorous quality standards.

The unprecedented convergence of Internet of Things ecosystems, edge computing, and generative artificial intelligence is reshaping the operational foundations of contemporary financial technology systems. FinTech platforms that increasingly rely on real-time analytics, personalized decision engines, and autonomous generative services face a fundamental dilemma: how to maintain privacy, security, and regulatory compliance while simultaneously delivering ultra-low latency and high-throughput intelligent services at scale. Cloud-centric models, once the dominant paradigm, are proving structurally inadequate for this emerging class of applications because of their inherent dependence on centralized data aggregation, excessive communication delays, and vulnerability to large-scale breaches. In response to these systemic limitations, distributed edge intelligence has emerged as a viable architectural alternative, enabling computational intelligence to be embedded closer to data sources while preserving operational efficiency and privacy.

This study develops a comprehensive theoretical and analytical framework for edge-AI microservice orchestration in privacy-sensitive real-time generative FinTech environments. The conceptual foundation of the article is grounded in recent advances in microservice-based Edge-AI architectures for private financial applications as articulated by Hebbar, Sharma, and Maheshkar (2026), whose work establishes orchestration, model isolation, and local inference as central pillars of trustworthy generative FinTech systems. Building on this foundation, the present research integrates a wide spectrum of scholarly perspectives from fog computing, secure IoT communications, blockchain-enabled trust management, federated learning, and cooperative edge resource management to construct a unified interpretive model of distributed financial intelligence.

Through an extensive theoretical synthesis of prior work, this article demonstrates how decentralized orchestration mechanisms transform FinTech services into dynamically adaptive ecosystems in which generative models, transaction processors, and anomaly detectors operate as loosely coupled yet cryptographically verifiable microservices. These microservices can be securely deployed across heterogeneous edge environments, enabling personalized financial intelligence while preventing unauthorized data leakage and systemic exposure. The article further argues that privacy-preserving generative finance is not merely a technological challenge but also an epistemic shift in how financial intelligence is produced, validated, and trusted.

By interpreting empirical and conceptual insights from blockchain-based access control, fog-enabled workload distribution, anomaly detection, and federated reinforcement learning, this study positions edge-AI microservice orchestration as the structural backbone of future FinTech infrastructures. The analysis reveals that such architectures fundamentally reconfigure economic agency by enabling financial decision-making to occur autonomously at the periphery of digital networks, thereby reducing dependency on centralized platforms while enhancing resilience and compliance. The article concludes that the integration of Edge-AI orchestration and privacy-aware generative modeling represents a paradigm shift that will define the next generation of financial computing.

Rectocele combined with cystourethrocele represents a frequent and clinically complex manifestation of pelvic floor dysfunction. Traditionally, these entities have been interpreted as isolated compartmental defects requiring local anatomical correction. However, accumulating anatomical, functional, and radiological evidence suggests that combined rectocele and cystourethrocele reflect a multilevel failure of the pelvic floor support system rather than a single-site structural abnormality.

This narrative review analyzes contemporary literature addressing anatomical interactions between anterior and posterior compartments, the biomechanical role of the rectovaginal septum and pubocervical fascia, levator ani dysfunction, and functional alterations detected by defecography, high-resolution anorectal manometry, and dynamic magnetic resonance imaging. The reviewed data demonstrate that symptom severity frequently does not correlate with the morphological extent of rectocele. Functional disturbances, including impaired anal relaxation, altered pressure gradients, and disrupted load redistribution within the pelvic floor, appear to play a decisive role in clinical presentation.

The concept of multilevel pelvic floor failure provides a more coherent framework for understanding the coexistence of rectocele and cystourethrocele. Damage to muscular and fascial structures, age-related connective tissue changes, chronic increases in intra-abdominal pressure, and obstetric trauma interact in a cumulative manner, leading to progressive destabilization of pelvic support. This systemic perspective explains the limited durability of isolated defect correction and underscores the importance of functional assessment in clinical decision-making.

Recognition of rectocele combined with cystourethrocele as a manifestation of integrated pelvic floor failure may refine pathogenetic interpretation and serve as a conceptual basis for further development of reconstructive strategies aimed at restoring biomechanical balance rather than correcting isolated anatomical deformities.

Chemotherapy-induced cardiotoxicity remains a major clinical challenge in patients with leukemias, contributing to reduced treatment tolerability and adverse long-term cardiovascular outcomes. Oxidative stress and neurohumoral dysregulation are key mechanisms underlying myocardial injury, while genetic variability may partly explain interindividual differences in susceptibility. This study aimed to evaluate the association of SOD2 (Val16Ala, rs4880) and COMT (Val158Met, rs4680) polymorphisms with the risk of cardiotoxic complications in leukemia patients receiving anticancer therapy. A total of 102 patients were stratified into subgroups with (n = 64) and without (n = 38) cardiological complications; 97 healthy individuals served as controls. Genotyping was performed using polymerase chain reaction with restriction fragment analysis. The SOD2 A allele showed a moderate association with cardiotoxicity, particularly for the homozygous A/A genotype (OR = 3.33; p = 0.12). In contrast, the COMT A (Met) allele demonstrated a strong and statistically significant association with cardiological complications (OR = 3.82; 95% CI: 1.97–7.42; p < 0.001), and the A/A genotype was associated with more than a fourfold increase in risk (OR = 4.45; p = 0.008). Combined analysis revealed a dose-dependent additive effect: carriage of two or more unfavorable alleles was associated with a more than fourfold increase in cardiotoxicity risk (OR = 4.22; p = 0.009). These findings support the role of pharmacogenetic factors in myocardial susceptibility and highlight the potential value of integrating SOD2 and COMT genotyping into personalized cardiovascular risk stratification strategies in leukemia patients.

Background: Diabetic foot syndrome (DFS) combined with ischemic heart disease (IHD) represents a high-risk vascular condition frequently requiring endovascular revascularization. However, clinical outcomes remain heterogeneous, particularly in the presence of diffuse arterial disease.

Objective: To evaluate angioarchitectural and procedural determinants of limb loss after endovascular treatment in patients with DFS and concomitant IHD.

Methods: A retrospective single-center study included 58 patients with DFS and IHD who underwent endovascular intervention between 2015 and 2019. Patients were stratified according to clinical outcome into favorable and unfavorable groups. Angiographic parameters analyzed included number of affected arterial segments, level of involvement, vascular access strategy, and type of intervention (balloon angioplasty alone or combined with stenting). Primary endpoint was major amputation. Secondary endpoints included procedural complications and early mortality.

Results: Unfavorable outcomes occurred in 37.9% of patients. Multi-segment arterial involvement (≥2 segments), extensive disease affecting ≥3 segments, and crossover vascular access were significantly more frequent in the unfavorable group (p <0.01). Combined balloon angioplasty with stenting was required more often in these patients (59.1% vs 33.3%, p <0.05). Stent occlusion, thrombosis, and early restenosis were associated with higher amputation rates. Major amputation occurred in 45.4% of patients with unfavorable outcomes, while early mortality reached 13.6%.

Conclusion: Diffuse angioarchitectural involvement and procedural complexity are strongly associated with limb loss after endovascular treatment in patients with DFS and IHD. Incorporating anatomical severity into preprocedural assessment may improve risk stratification and limb salvage strategies.

Background: Laparoscopic mini gastric bypass is widely performed for the treatment of morbid obesity due to its metabolic efficacy and technical simplicity. However, concerns remain regarding postoperative bile reflux associated with its loop configuration.

Objective: To evaluate the incidence and clinical significance of bile reflux following conventional laparoscopic mini gastric bypass in a single-center cohort.

Methods: A retrospective analysis was conducted on 68 consecutive patients who underwent con-ventional mini gastric bypass between 2021 and 2023. Patients were stratified according to pre-operative gastroesophageal reflux status. Early postoperative outcomes were assessed at 30 days, including endoscopic visualization of bile in the gastric pouch and reflux-related symptoms. Mid-term outcomes at 12 months included excess weight loss (%EWL), reflux dynamics, and quality-of-life evaluation.

Results: At 30 days, bile was visualized endoscopically in 90.6% of patients with preoperative re-flux and in 47.2% of those without prior reflux (p < 0.001). De novo reflux symptoms developed in 27.8% of previously asymptomatic patients. Most patients in both subgroups achieved ≥50% EWL at 12 months, with greater weight loss seen in those without preoperative reflux.  Complete resolution of reflux symptoms occurred in 18.8% of patients with baseline reflux, whereas persis-tent or partially regressed symptoms remained common. Quality-of-life improvement was observed overall but was lower in patients with ongoing reflux manifestations.

Conclusion: Conventional mini gastric bypass ensures effective weight loss but is associated with a high incidence of early bile reflux, particularly in patients with preoperative gastroesophageal re-flux disease, and not negligible even in initially asymptomatic individuals.

A 38-year-old male patient, A.H., sustained multiple injuries, including a femoral fracture with damage to the superficial femoral artery (SFA) and fractures of the right 1st to 8th ribs, after falling from a height of four meters during a handstand. The patient underwent simultaneous surgical interventions, including crossectomy of the great saphenous vein, autovenous grafting of the SFA to restore vascularization, and retrograde intramedullary locked nailing of the femur. After four weeks, the patient was discharged from the hospital, bearing weight on both lower limbs with the assistance of a walker.
